The JGH Resource Center was established in September 2007 to serve as a point of origin to help the public better understand the many options available to them as they navigate through what can sometimes be a complex and challenging healthcare system.
Jackson General Hospital has provided Case Management Services to inpatients for many years. This includes coordination of care for individual health needs, referrals to outside agencies such as Home Health, Rehabilitation Hospitals, Skilled Nursing Homes, Long Term Care, Assisted Living, Private Duty Care, etc. and provision of information about financial assistance for those who qualify. Recognizing the uninsured and underinsured patients, as well as the stress associated with the complexity of health care options, particularly for the elderly, we are extending this service to the outpatient population as well.
Case Management Staff will be providing the following services through the Resource Center:
Financial assistance information
Social assistance information
Coordination of medical care
Helping those in need
Each medical unit in the hospital has an assigned caseworker to assist you and your family with services in the community or the hospital. We are available to answer questions such as completing a Health Care Proxy or advance directives, insurance concerns, specialized programs to treat substance abuse problems, Hospice, long-term care nursing home placement, and various post-hospital services.
The case management team can be reached by calling ext. 1562 between the hours of 7:30 a.m. and 4:30 p.m., Monday through Friday, or by asking your nurse to contact the assigned worker for your floor during weekends and holidays.
